Step 3: Referral Details

During the third step of case creation, the user will provide the details of the case referral, including the requirement for which the case was referred to be fulfilled (for example, a successful return to the workforce, or a specific variety of service) and the identity of the individual or company responsible for referring the client.

The referral details fields allow the user to specify the case requirement, along with the date on which the client was referred, the client's status before their referral (for example, unemployed, or recovering from injury), and the source of the referral itself.

The Referrer Search allows the user to streamline this step by searching for any prior referrers and selecting them as the referrer for the case currently being created. As part of this step the user is also able to click the 'Employer' button to set the case referral details to be identical to the client's employer details. This is useful in the circumstance of a company which has referred a member of its own staff as the case client and prevents the need to re-enter the same details when specifying the client's employer, and the case referrer.

  • Reason for referral

The required service, treatment or outcome for which the client was referred.

  • Reason for referral (personal)

This field is used when a personal reason is the cause for the referral.

  • Reason for Referral (work related)

The required service, treatment or outcome for which the client was referred.

  • Referred On

The date on which the client was referred.

  • Acceptance Date

The date that the client was accepted for treatment

  • Delay to Commencement

The time differential between the date the client was referred and the date the client was accepted for treatment.

  • Client Status Prior To Referral

The status of the client at the time of their referral with regards to their injury or the cause for their referral - for example, if the client was hospitalized, or temporarily incapacitated it would be noted in this field.

  • Client Status of Case Closure

Employment Status of the client at conclusion of case. Client could be with the same or new employee, same or new role or not employed and on benefits, etc.

  • Source of Referral

The individual or organisation responsible for referring the client.

  • Date Ceased Work

The date on which the client ceased their previous employment.

  • Referrer

The identity of the company responsible for referring the client.

  • Contacts

When the Referrer field has been entered the Iinsight system will immediately check its records for any contacts previously associated with this company or contact name. If any are found, they will be listed in the Contacts drop-down and made available to the user. Clicking one of these contacts will populate the Contact Details fields with the selected contacts information.

  • The 'Same As Employer' Button

It is common for a client to be referred by their employer in the circumstance of a workplace injury. In this circumstance, the user is able to copy all of the client's employment contact information to serve as their referral contact information by clicking the 'Same as Employer' button.

  • The 'Clear' Button

Clears all of the Contact Details fields and de-selects any contact chosen in the 'Contacts' field.

  • Company

The identity of the company with which the client's referral contact is affiliated.

  • Title

The title, or salutation, of the client's referral contact.

  • First Name

The first name of the client referral contact

  • Last Name

The surname of the client's referral contact.

  • Position

The position with their organisation of the client's referral contact.

  • Department

The department of their organisation with which the client's referral contact is affiliated.

  • Address, Suburb, State, Postcode

The postal address of the client's referral contact. This is typically the postal address of the organisation with which they are affiliated.

  • Email

The contact email address of the client's referral contact.

  • Phone 1

The primary contact telephone number of the client's referral contact.

  • Phone 2

The secondary contact telephone number of the client's referral contact.

  • Fax

The fax number of the client's referral contact.

  • Mobile

The mobile telephone of the client's referral contact.

  • Job Title

The job title of the client's referral contact. This is primarily used to ensure that automatically generated documentation contains accurate content.

  • Case Notes

Notes related to the client's referral contact, which will be stored permanently alongside the case entry.
